effective treatment plan. Our nursing professionals are committed to excellence in home
health care. Many elements are involved in a successful treatment plan, and we pay careful
attention to provide both a coordinated and effective care plan. Our nursing staff is keenly
aware of their critical role in the patient’s medical care. We recognize that the medical field
is continually changing and making new discoveries, especially in the fields of wound care,
geriatrics, Parkinson’s Disease, Alzheimer’s Disease, and pain management.

Physical Therapy
Physical Therapists specialize in treatments that improve the mobility and physical strength of patients. Physical therapy primarily consists of physical exercise to alleviate pain and restore muscles along with patient teaching of special techniques to improve mobility. Physical therapy is delivered in accordance with physician’s orders and as defined in a plan of care.
The following are examples of physical therapy provided in the home:
- 1. Assisting with ambulation, transfers, muscle weakness, tremors, spasticity or joint stiffness
- 2. ROM (range of motion) and positioning
- 3. Exercise programs
- 4. Assistance with respiratory problems
- 5. Assistance with skin problems

Occupational Therapy
Occupational Therapists specialize in treatments that enable patients to perform the general activities of daily living (ADL’s) such as bathing, eating, dressing and basic household routines. Generally individuals who have physical, developmental and/or social problems might need the skills of an Occupational Therapist. Occupational therapy is delivered in accordance with physician’s orders and as defined in a plan of care.
The following are examples of occupational therapy provided in the home:
- 1. Assist with safety concerns that may stem from sensory impairment
- 2. Assist with perceptual motor difficulty, coordination problems, and muscle weakness
- 3. Assist with energy conservation

Home Health Aide
Home Health Aides (HHA) assist patients in their completion of activities of daily living (ADL’s) and personal care. Some are specially trained and competent to deliver more complex services under the direction of a registered nurse. Often the HHA can be the primary care giver due to frequency and duration of their visits.
The following are examples of home health aide care provided in the home:
- 1. Assists with bathing, hair care, skin care, dressing, oral hygiene and meal preparation
- 2. Assists with bowel and bladder functioning
- 3. Assists with ambulation and range of motion activity; obtains vital signs

Speech Therapy
Speech and language pathologists specialize in treatments that aid patients to restore speech that may have been lost due to some form of trauma, for example a stroke. Speech Therapists also train patients to improve their breathing, swallowing as well as their speech. Speech therapy is delivered in accordance with physician’s orders and as defined in a plan of care.
The following are examples of speech therapy provided in the home:
- 1. Assist with communication difficulty
- 2. Treats swallowing problems and the side effects that are associated with the swallowing disorder
- 3. Improve and restore lost language function and ability

Medical Social Worker
Medical Social Workers specialize in evaluating the social and emotional factors that effect the ill and disabled. They help patients and their families manage the stress that can be put on families and connect them with the necessary community resources available to them. Medical Social Work is delivered in accordance with physician’s orders and as defined in a plan of care.
The following are examples of medical social worker services provided in the home:
- 1. Assists with homemaker-companion needs
- 2. Assists with relationship problems, anxiety / depression, and finances
- 3. Offers education regarding community resources
- 4. Offers information regarding alternative living arrangements
- 5. Assists with psycho/social issues and the effects of long term illnesses
